The head should be epoxied to the spring. I'd pull it and check for cracks around the bolt holes. I just found one with cracks on both sides so we replaced the head and glued it on. Your service manual might have the epoxy info but off the top of my head it's Loctite NA9309?
 
Yes I have seen this.  Best case it’s the bolts, next best case is wear in the tailwheel head, worst case wear in the stinger holes.

I’d put new bolts in first. It’s worth a shot.  Sometimes just the CAD plating that’s worn down can cause some slop.
